We’re excited to welcome you to the 2026 Parkinson's Policy Forum! Please complete the form below to confirm your attendance. Your responses will help us coordinate accommodations and logistics and ensure your experience is as smooth and impactful as possible. 

Important note: Each attendee must register individually — that includes any family members or care partners who plan to attend! Please also enter your care partner or family member’s name on your registration form so we know you’re attending together. That helps us coordinate hotel rooms and congressional meetings accordingly.

Please note registration confirms attendance. If you register and later find out you cannot attend, please let us know ASAP so we do not schedule meetings for you.
